Build survey protocol into Question Pro

Directions
  1. UnderCreate the current year's folder. Hit the bluea New Survey button
    1. Log in to QuestionPro
    2. Click “New Survey.”
    3. Name it the yearsurvey andusing project nameYear + hitProject Create Survey.
    4. Begin adding questions from the protocol. For the intro and any standard protocol questions, go to Question Library under Workspace, select the project you want to pull it from (e.g., Standard Survey) and select the box next to the ones you want to add. 
    5. For all questions, make sure they match the approvedName format (e.g., comment2025 box,APS selectWorkplace one,Study).

  2. Add anyStandard logicDORIS indicatedOpening in the protocol. 
  3. After inputting all the questions, go to Preview mode to ensure it is error-free + any display logic is performing correctly. 
  4. After making any corrections, click the link in the upper right hand corner of the page. This is the Open Link. Copy/paste it into the Project Charter and Project Overview doc in the project's Interviewer Packet on Google Drive and Text

    6. Select the instructions and opening questions you want to import.
    7. Click “Add Selected Questions” — they will populate into your current project.
  5. InformAdd Questions from the OpsInterview/Survey ManagerProtocol
    1. Open the project protocol isdocument entered(Google +Doc tab).
    2. Copy each question text.
    3. In QuestionPro, click “Add Question.”
    4. Choose the linkcorrect isquestion intype:
    5. Open-ended: “Comment Box”
    6. Select One / Multiple Choice: “Select One”
    7. Matrix / Grid: “Spreadsheet”
    8. Instructional / Divider: “Presentation Text”
    9. Paste question text into QuestionPro.
  6. Code Each Question
    1. In the CharterCode +field, Interviewercreate Packet.a short marker (1–2 words) summarizing the question’s topic (e.g., about-you, office-collab, one-change).
    2. This helps with tracking and logic setup later.
  7. Add Response Options (for Select One Questions)
    1. Enter each option (e.g., Home-Based, Hybrid, On-Site).
    2. Add an “Other” field with a Dynamic Comment Box for custom responses.
  8. Add Logic (Skip Logic / Display Logic)
    1. Click the Logic tab.
    2. Select “Skip Logic” → “Show/Hide Question.”
    3. Identify which question(s) to show/hide based on previous responses (e.g., job classification or work mode).
  9. Add DORIS “Modes of Engagement” Section
    1. Insert a Presentation Text block for introduction text (from the protocol).
    2. Copy/paste the paragraph beginning “DORIS understands work through four modes of engagement...”
    3. Add a Spreadsheet question to capture the time allocation table:
    4. Rows: Personal, Transactional, Recovery, Relational
    5. Column: “Percentage of time”
    6. Set column total = 100 under “Settings.”
  10. Add Closing Text
    1. Add Presentation Text: “Please answer the following questions” or “Thank you for your valuable input!”
    2. Include DORIS logo if applicable.
  11. Preview and Test
    1. Click “Preview” (eye icon) to view full flow.
    2. Click “Start” → “Next” → “Done” to test navigation, logic, and required fields.
    3. Confirm that all imported text appears as expected and logic performs correctly.
  12. Review and Finalize
    1. Share preview link with Lead Project Strategist for review and content validation.
    2. Make any requested adjustments.
    3. Save and publish survey for testing or distribution.
