Build and edit survey protocol into Question Pro

“Survey Template 2025” is published and accessible under the DORIS QuestionPro account.
Directions
- Create a New Survey
- Log in to QuestionPro
- Click “New Survey.”
- Name the survey using Year + Project Name format (e.g., 2025 APS Workplace Study).
- Click “Create Survey.”
- Log in to QuestionPro
- Add Standard DORIS Opening Text
- Click “Add Question.”
- Open the Question Library.
- Navigate to My Surveys → Survey Template 2025.
- Select the instructions and opening questions you want to import.
- Click “Add Selected Questions” — they will populate into your current project.
- Add Questions from the Interview/Survey Protocol
- Open the project protocol document (Google Doc tab).
- Copy each question text.
- In QuestionPro, click “Add Question.”
- Choose the correct question type:
- Open-ended: “Comment Box”
- Select One / Multiple Choice: “Select One”
- Matrix / Grid: “Spreadsheet”
- Instructional / Divider: “Presentation Text”
- Paste question text into QuestionPro.
- Code Each Question
- In the Code field, create a short marker (1–2 words) summarizing the question’s topic (e.g., about-you, office-collab, one-change).
- This helps with tracking and logic setup later.
- Add Response Options (for Select One Questions)
- Enter each option (e.g., Home-Based, Hybrid, On-Site).
- Add an “Other” field with a Dynamic Comment Box for custom responses.
- Add Logic (Skip Logic / Display Logic)
- Click the Logic tab.
- Select “Skip Logic” → “Show/Hide Question.”
- Identify which question(s) to show/hide based on previous responses (e.g., job classification or work mode).
- Add DORIS “Modes of Engagement” Section
- Insert a Presentation Text block for introduction text (from the protocol).
- Copy/paste the paragraph beginning “DORIS understands work through four modes of engagement...”
- Add a Spreadsheet question to capture the time allocation table:
- Rows: Personal, Transactional, Recovery, Relational
- Column: “Percentage of time”
- Set column total = 100 under “Settings.”
- Add Closing Text
- Add Presentation Text: “Please answer the following questions” or “Thank you for your valuable input!”
- Include DORIS logo if applicable.
- Preview and Test
- Click “Preview” (eye icon) to view full flow.
- Click “Start” → “Next” → “Done” to test navigation, logic, and required fields.
- Confirm that all imported text appears as expected and logic performs correctly.
- Review and Finalize
- Share preview link with Lead Project Strategist for review and content validation.
- Make any requested adjustments.
- Save and publish survey for testing or distribution.
